how does it know the chain?

it asks all 12 chains at once and uses the one that has the hash. hashes are practically unique across chains.

where does the revert reason come from?

failed txs don’t store their error, so it replays the call with eth_call at that block and decodes what comes back. if the state changed since, the replay may succeed instead.
